The application forms to obtain birth, marriage, and death certificates are available by phoning and having them mailed or faxed to you, or by visiting any of the offices. (Please note: Forms will only be faxed to an Ontario fax number.) Or from this Web site: https://www.orgforms.gov.on.ca/eForms/start.do

Application forms can be mailed or faxed (807-343-7459), or brought into one of the offices. If faxing applications, provide type of credit card for payment (Visa or MasterCard), the credit card number, the expiry date, and the full name on the credit card.
